I have read and understood The Paw Valley's hours of operation listed above. I understand that a fee of
$1.00 dollar per minute
(Maximum 30 mins) will be charged for late pickups, and late drop offs. If I fail to pick up my dog(s) within 30 minutes past the closing time, my dog will be assigned to overnight boarding, to be picked up in accordance with the new respective pick up time the following day, with all encompassing charges added ($1.00 per minute for 30 minutes + applicable boarding fees). I understand that the only clock used to determine our hours of operation & applicable late fees is our office clock.
I understand that The Paw Valley’s updated hours of operations can be found anytime at pawvalley.ca/prices-hours. I understand these hours are general, and are subject to change at any time, and I agree it is my responsibility to ensure that I understand what times certain actions may be required of me (such as dropping off your dog(s), or picking them up) in order to use the services on the given day(s).
General Requirements to Access The Paw Valley’s Services:
In order to utilize The Paw Valley’s services, I understand that I must fully complete all 3 enrollment forms and go through the The Paw Valley assessment process. All dogs must have complete, up-to-date, and approved enrollment forms on file. By signing, I agree that I have authority to sign on behalf of and have obtained express approval from all participating owners of my dog(s).
I understand that I must thoroughly read, complete and sign the:
1. Registration Form (completed online)
2. The Paw Valley Policies (This Document)
3. The Paw Valley Service Agreement & Liability Waiver.
By utilizing The Paw Valley’s Services and signing the forms, I agree that I have read it in its entirety, understand its contents completely and agree to follow the set terms in order to access The Paw Valley’s Services. I also understand that no dog will be admitted into our care unless the above 3 forms are fully understood, executed and verified in our systems.
Regular Daycare Attendance and Account Inactivity:
I understand that I must be active in the The Paw Valley Program in order to access The Paw Valley's Services. I understand that in order to stay active in the The Paw Valley Program, attending daycare or boarding at least once every week is mandatory. I understand it is entirely my responsibility to keep my account active and in good standing to continue using The Paw Valley's Services. The account expiration date is calculated as 7 calendar days from the last time my dog(s) were participating in The Paw Valley's Daycare or Boarding Services. I also understand that should I be removed from the program, daycare pass expiration dates still apply and will not be extended, transferred or refunded for any reason. In order to rejoin the The Paw Valley program, I understand I must rejoin the waitlist and re-complete the assessment process, subject to availability, current waitlist length and the sole discretion of The Paw Valley. Please note, the 7 day count includes days The Paw Valley is closed for daycare, such as weekends, but does not include statutory holidays in the count. The only exception to this Attendance Requirement is for certain cases for clients who have received express consent to attend once every two weeks (14 calendar days), granted prior to January 1st 2025; in these cases the Attendance Requirement stipulations all still apply, with the exception of 14 calendar days rather than 7.
Bookings, Cancellation & Refunds:
I understand that bookings are on a first-come, first-served basis and are required for all daycare & boarding stays. Further, I understand that bookings must be made through pawvalley.ca ahead of time, and The Paw Valley does not allow drop-in visits. I understand that The Paw Valley cannot reasonably guarantee services and reserves the right to cancel any booking for any reason if required. Cancellation requests are fully refundable for daycare bookings up to 24 hours before the booking begins. For cancellation requests made within 24 hours of the booking start time, a $5 fee will apply for daycare cancellations, and a 10% charge of the total boarding cost will be applied. Failure to attend a booking without any notice prior to booking start time will result in no refund at all for the booked service (Both daycare & boarding). Any cancellation/refund must be within reason & is completely at the discretion of The Paw Valley's staff. Please note that refunds are given through store credit by default, and refunds to original payment method are available upon request and are subject to a 7% fee. I agree to double check the dates, the price, and all other related conditions are correct before completing my booking.
Daycare Packages (Play Passes):
I understand that the 5 and 10 day (1 & 2 dogs) passes
expire 60 days from the date of purchase

30 days from purchase
(including any closed daycare days that may occur during that period of time, such as weekends and statutory holidays). I understand that
passes are non-refundable, non-transferable and non-extendable by any circumstances and can only be used for the dog it was originally purchased for
. I also understand that if I am removed from the program for any reason, the original pass expiry date still stands, and will still be non-refundable, non-transferrable and non-extendable. I agree that it is my responsibility to keep note of my purchased passes expiration dates.
Vaccinations, Neutering & Flea/Tick Prevention:
I understand The Paw Valley has the following requirements:
- All dogs remain current with DHPP, Rabies & Bordertella vaccines.
- All dogs over 8 months old be either spayed or neutered.
- Vaccinations must be administered by a registered vet.
- All dogs must be on a flea & tick preventative between May 1st to October 31st in order to protect all dogs from pests in our large outdoor play space. I understand that I may be asked to provide proof of flea prevention (either product packaging or vet invoice) before utilizing The Paw Valley's Services during this period of time. The Paw Valley does not allow flea collars as they are toxic and hazardous to dogs if bitten or chewed. It is my, the owner’s, responsibility to ensure the correct precautions are being taken to protect my dog(s) from fleas, ticks and other insects/bugs. The Paw Valley has a massive outdoor area, and despite being maintained, it is impossible to guarantee it is bug free, thus making preventatives incredibly important for the health of my dog(s), myself, and staff.
- All puppies must have their second round of vaccinations.
- Dogs’ recently vaccinated/boosted must quarantine for 72 hours before attending The Paw Valley.
I understand that I am responsible for providing The Paw Valley with an up-to-date vaccination certificate(s) from my vet verifying these vaccinations are current. The Paw Valley reserves the right to limit facility usage by members with expired vaccinations until valid proof of renewal is received. I understand that failing to vaccinate my dog(s) on time is very dangerous in a group dog setting, and I agree that I am wholly responsible for damages from illness/injury to my dog and others if an issue is to occur from non-vaccination.
Privacy & Property:
I acknowledge and understand that while The Paw Valley operated commercially and property is fully permitted as required by the City of Edmonton, it is still a residential area. As such, I agree to respect the specified hours of operation for all services provided by The Paw Valley. Just like any home, however with the added responsibility of taking care of our clients precious dogs, we take our property's privacy and security very seriously. Under no circumstances will any overnight workers facilitate the arrival or departure of dogs from The Paw Valley's care. All dog arrivals and departures must adhere strictly to the established hours of operation and check-in/check-out procedures as outlined in this agreement unless explicit written permission is provided by The Paw Valley. By agreeing to The Paw Valley Policies, you firmly agree that any presence on the property outside of our hours of operation is in direct violation constitutes trespassing and may result in police intervention, potential legal charges & potential civil litigation.
Health Requirements:
All dogs must be in good health. On admission, all dogs must be free from any condition that could potentially jeopardize other guests or themselves. We will not admit any dog that we believe, at our sole discretion, to be a risk to the other animals in our care. It is your responsibility as a parent to notify us of any health related issues affecting your dog so that we can make an informed determination about how to best care for your dog.
Please do not bring your dog to The Paw Valley if they have diarrhea, a cough, vomiting, fever, worms, fleas, or have exhibited any signs of an infectious disease or any other illness or injury. If your dog is found to be sick, you or your emergency contact will be notified and your dog will be isolated until you can pick him or her up. Depending on the severity of the illness, we may also take your dog to the vet for further diagnosis and treatment. Please note that as the owner you will be responsible for all medical and veterinary costs incurred.
Behaviour & Temperament:
All dogs utilizing our daycare or overnight stays services are temperament tested for suitability to socialize and play with other dogs. This helps to ensure the safety of all of the dogs in our care. All dogs must be non-aggressive and non resource guarding. You will need to certify that your dog(s) have not harmed or shown any aggressive or threatening behavior towards any person or any other dog(s). Please remember that your dog will be spending time in an open environment with other dogs and the safety and health of all of our guests is our main concern. The Paw Valley reserves the right, without explanation, to restrict usage to any dog that it deems, in its sole discretion, to be unsuitable for our daycare or overnight stay services.
Emergency Procedures:
In the event of injuries to dogs, I understand that The Paw Valley's emergency procedures are structured to ensure swift and appropriate actions are taken. Injuries are categorized into three color codes, each determining the necessary action plan for treatment.
1. Yellow (Mild): This category covers mild injuries such as small scratches, minor bruises, superficial skin irritations, or other minor injuries that do not require immediate veterinary attention. Treatment is usually applied on-site by The Paw Valley's trained staff, and I will be notified during the day, typically at pickup time.
2. Orange (Serious): This designation is for more serious injuries requiring veterinary intervention, including cuts/scrapes showing signs of infection, excessive bleeding that persists, injuries causing notable behavioural changes, persistent limping, vocalization of pain, signs of distress, or any condition that may worsen without prompt veterinary assessment. The Paw Valley's staff will attempt to address the situation and promptly contact the primary and, if unreachable, emergency contact to discuss the next steps.
3. Red (Emergency): The most serious colour code indicates an emergency situation requiring immediate vet care. This includes severe wounds, uncontrolled bleeding, difficulty breathing, loss of consciousness, seizures, ingestion of toxic substances, or any critical condition requiring immediate and intensive veterinary care. In such cases, The Paw Valley's staff will promptly transport my dog to the nearest emergency vet and make diligent efforts to reach the primary or emergency contact.
By signing below, I provide The Paw Valley with express permission to handle injuries using the most appropriate means as determined solely by The Paw Valley's staff. This includes taking your dog to a vet without prior consent if deemed necessary by The Paw Valley's staff. All financial expenses related to veterinary care are solely my responsibility. I also understand that while knowledgeable in dogs and possessing basic first aid training, The Paw Valley staff are not veterinarians, and thus I agree and understand that the staff’s initial assessment of extent of injury and treatment of injury might not be fully accurate, and thus are not liable for damages arising directly from the treatment or failure to treat any injury/sickness. My signature acknowledges my understanding and agreement with The Paw Valley's emergency procedures and any methods they deem fit for treating my dog.
Payments:
I understand that payment must be made for goods and services rendered & scheduled to be rendered at a future date. The Paw Valley accepts Visa, MasterCard, debit, bank e-transfer, and cash payments. The Paw Valley reserves the right to terminate services or suspend memberships if unpaid balances exist and sufficient notice of unpaid balances has been given.
Leashes, Collars & Identification Tags:
All dogs are required to have a quick release collar with proper identification tag. The tag must be attached to my dog’s collar with a legible name and phone number. I will ensure that the collar is well fitted and that my dog cannot slip out of the collar. The Paw Valley does not accept choke chains, prong/pinch collars or shock collars at their facility. Dogs must be transported to and from The Paw Valley with their own leashes. The Paw Valley does not take any responsibility for any damage or loss that may occur to your dog’s collar or leash while in their care. Things get misplaced, taken by other owners by accident, or damaged/destroyed in the daycare environment quite often, please do not bring any collar/tags with significant value (monetary or emotional).
Toys, Blankets & Beds:
The Paw Valley will provide my dog with all the amenities they need including a dog bed, food bowls, toys and blankets! Please do not bring your dog’s personal belongings from home, if you do decide to bring your own, you acknowledge and accept the risks involved with bringing the items to a daycare environment (misplaced, damaged, lost, etc).
Food & Medication:
I understand that I must package my dog’s food in individual meal-size portions for boarding stays for stays shorter than 7 nights. Each package should represent one meal, and each meal should be clearly marked with my dogs’ name and meal timing. Pre-packing meals in this fashion ensures that the food supply is not depleted prematurely and that my dog is fed the usual amount during their stay! Failure to adhere to this policy may result in an additional charge of $1 per meal fed during the stay. I will also throw in an extra few meals, just in case I am delayed in picking up my dog(s). For stays over 7 nights, please bring your dogs’ food in the one or more packages/containers, with a scooper and instructions on feeding (how many scoops? How many meals per day? When?).
The Paw Valley is able to administer any oral or topical medication, however they cannot administer any injections. All medication needs to be labeled, with my dog’s full name and instructions on administration. I will bring at least two extra days of medication just in case I need to extend my stay. In addition, I will carefully review medication instructions with a The Paw Valley staff member at the time of reservation and check-in. If The Paw Valley is unable to serve the medication, they will contact me first to review options, potentially including consulting a veterinarian. Any resulting charges are my responsibility and are due before pick up. Please remember to collect all unused medications at the time of pick-up.
Holiday Season Boarding Altered Terms:
For boarding reservations that include any dates within the date range of December 20th, to January 2nd :
- Cancellation requests sent 72 hours or more prior to start date & time may receive a full refund.
- A cancellation request made between 72 hours and 24 hours before booking start date & time are only entitled to a maximum refund of 50% of the total paid amount.
- A cancellation request made within 24 hours of booking start time is entitled to a maximum refund of 25% of the total paid amount.
- A no show, or a client who does not attend their booking for any duration of time past the start time will not be refunded any amount under any circumstances.
- Any cancellation/refund must be within reason & is completely at the discretion of The Paw Valley's staff. Please note that refunds are given through store credit by default, and refunds to original payment method are available upon request and are subject to a 10% fee. I agree that I will double check the dates I’m looking to book and ensure they are correct before completing my online booking!
False/Unfaithfully Represented Information:
As a professional dog care service provider, The Paw Valley expects all clients to provide accurate and truthful information about their dogs. If I am found to have lied or unfaithfully represented any information about my dog(s), whether in writing or verbally, The Paw Valley reserves the right to terminate its services with immediate effect and without refund. Furthermore, I will be liable for any damages or losses incurred by The Paw Valley as a result, whether direct or indirect, of their misrepresentation, including but not limited to veterinary bills, damage to property, and legal fees.
I certify that I have read and understand the policies set forth on the preceding text and that I have read and understand this agreement. I agree to abide by The Paw Valley's Policies and accept all the terms, conditions, and statements of this agreement.
By signing below, I agree to enforce this agreement (The Paw Valley Policies) with the same signature given at the end of the Registration Form, indicating that I agree that I have read and understand this agreement. I also agree that the signature will be the electronic representation of my signature for all purposes when I use them on documents, including legally binding documents, just the same as a pen-and-paper signature.

Service Agreement & Liability Waiver
I hereby enter this agreement/waiver with The Paw Valley and agree to the following terms and conditions of my dog(s)' participation in services at The Paw Valley.
In consideration for your dog(s) being permitted to come to The Paw Valley' facilities or use any of The Paw Valley' services, you agree to all the policies, procedures, release, terms, and conditions set forth below. The following Agreement/Waiver will apply to The Paw Valley inc., its directors, officers, members, managers, employees, contractors, agents, sponsors, organizers, and other representatives (hereafter collectively referred to as "The Paw Valley").
1. Liability Release: In consideration for the services provided by The Paw Valley, I hereby unconditionally release, discharge, and forever hold harmless The Paw Valley from any and all liability, claims, demands, actions, causes of action, costs, expenses, and damages whatsoever, whether in law or equity, whether known or unknown, arising out of or in connection with any injury, illness, loss, damage, or harm to my dog(s), myself, other dog(s), or any property, whether directly or indirectly caused by or resulting from:
a) Accidents, incidents, escape, weather, or other events that may cause harm/loss that may be beyond the control of The Paw Valley, despite reasonable precautions taken.
b) The inherent risks associated with group play activities, including but not limited to, rough play, bites, scratches, and other injuries, as well as illnesses such as kennel cough, or other diseases/illnesses obtained through any consumption or communicable means.
c) Inherent risks of outdoor play such as Ingestion of materials or substrates, potential encounters with wildlife such as biting or stinging insects or fleas and ticks, escape, & weather-related health issues such as allergies.
d) The aggravation of any pre-existing medical conditions that may be exacerbated by the excitement, exercise, or interactions associated with group play activities, boarding, or other services offered by The Paw Valley.
e) Any harm or injury caused by my dog(s) to other animals, persons, or property while in the care of The Paw Valley; to which I, myself, as the owner am solely fiscally responsible for damages.
2. Grounds for Denial of Service: I understand that The Paw Valley reserves the right to any deny service to dogs exhibiting temperament problems, including but not limited to selectiveness, excessive barking, aggression, excessive marking, destructiveness, excessive anxiety, resource guarding, mounting, stalking, or extreme shyness with dogs or people. I acknowledge that The Paw Valley may also refuse service to pets that appear sick or display behavior that could endanger their health, the safety of other pets, the staff, or any other individuals. I am aware that the refusal policy is entirely at the discretion of the The Paw Valley staff and does not necessitate an explanation. I further understand that if my dog displays any of these behaviors, illnesses, or issues during their time at The Paw Valley, they will be promptly removed from our group setting, and kept separate until pickup is possible. I recognize that The Paw Valley upholds a strict zero-tolerance policy for such cases. I agree that I will be refunded for the day of daycare or boarding (pro-rata), and I am responsible for picking up my dog(s) promptly. I acknowledge that in cases where unacceptable behavior persists, and my dog(s) are not retrieved within a reasonable time frame, refunds may become void, and additional charges may apply. Additionally, I am aware that The Paw Valley may deny future services based on my actions as the owner. Such actions may include chronic lateness, disregard for The Paw Valley' policies, disrespectful or rude behavior towards the staff, or any other behavior deemed detrimental to the well-being of the facility, the staff, or other clients and their pets. I understand that despite guidelines being in place, The Paw Valley retains the right to refuse services or admittance to any dog(s) for any reason, with or without explanation.
3. Emergency Veterinary Care: I agree that The Paw Valley may seek veterinary care for my dog(s) if such care is reasonably deemed necessary for the welfare of my dog(s). The Paw Valley agrees that they will make all reasonable effort to contact the Guardian(s) or Emergency contact and the treating veterinarian before seeking treatment. However, I agree that if such contact is not successful, or if the situation is an emergency that may not allow time for communication, that The Paw Valley may take reasonable steps to ensure the health and safety of my dog(s) by whatever means necessary under the sole discretion of The Paw Valley. Settlement of any fees/costs associated with any required care are solely my fiscal responsibility.
4. Health and Vaccination Certification: I certify that my pet is in good general health, free of contagious diseases or conditions, sound mental health and exhibit a reasonable disposition. I also confirm that I have fully disclosed all known health and behavior issues to the best of my knowledge during my dogs’ admission & continued usage of The Paw Valley. Further I will maintain up-to-date vaccinations and flea/tick preventative as requested by The Paw Valley. I acknowledge that I will adhere to The Paw Valley' recommendations regarding my pet's temperament, health, training, and socialization; even if that recommendation is to discontinue use of The Paw Valley services.
5. Media and Information Usage Consent: I consent to the unlimited & perpetual use of photos, videos, and recordings of my dog(s) as property of The Paw Valley for any use, such as, promotional and security purposes. This includes their publication on The Paw Valley' website and social media channels. I also grant permission for the use of images/video including myself, relevant parties, and relevant details, such as conversations, names, booking details for security, record-keeping, replying to public online correspondence and legal compliance. Any promotional/marketing use of content featuring my personal information will require separate approval, and I retain the right to withdraw consent for such use with reasonable notice.
6. Review and Opinion Clause: I have the right to share my thoughts about The Paw Valley both online & offline. I understand that if I deliberately try to harm the business, share false information, or act with malice, I will be personally liable. By signing below, I unequivocally accept and acknowledge this personal liability for possible damages caused to The Paw Valley. If there's a problem, The Paw Valley can send me a Cease & Desist letter, asking me to rectify the issue. If I don't fix it within a set time (not more than 14 days from served date), I confirm and accept full responsibility for potential legal action. The Paw Valley may ask for $150 per day for each day the damaging content exists publicly to compensate for damages, which may be direct monetary losses, or indirect such as reputational damages. By signing below, I signify my unwavering acknowledgement and acceptance of my personal liability for any breach of this clause.
7. Property-Related Risks and Liability Release: I acknowledge that The Paw Valley is located on an acreage in Edmonton, and its property characteristics may expose visitors to various potential risks, including but not limited to slip-and-fall incidents on icy surfaces, vehicle-related challenges during snowy conditions, accidents in the parking lot, slips on wet surfaces, or tripping over objects like rocks or sticks. While The Paw Valley endeavors to maintain the property, its vast size makes it impossible to anticipate every potential hazard comprehensively. Therefore, I assume full responsibility for my safety and the safety of my property while on the premises, releasing The Paw Valley from any and all liability for losses, damages, or injuries that may occur, whether directly or indirectly resulting from the property's characteristics or external factors.
8. I hereby waive and release The Paw Valley from any and all liability for injury, illness, or damage resulting from the actions of my dog(s), any other dog, or any human. I expressly assume the risk of any injury or illness to my dog(s) including any and all medical expenses resulting from or relating to said injury or illness.
9. I herby agree to indemnify and hold harmless The Paw Valley, its employees, officers, contractors, members, clients, agents, and guests from any and all claims resulting from any action(s) by any dog in the care of The Paw Valley, (including my dog(s)) and that this release and indemnification shall be in force whenever my dog(s) participates in any activities at the facility starting with the date indicated below and thereafter.
10. I acknowledge that The Paw Valley may, at its sole discretion, update or modify this agreement/waiver and any of its policies at any time. Updated copies of this document and other relevant documents will be made available on The Paw Valley' official website at playfulacres.ca/Documents. The Paw Valley will make reasonable efforts to provide notice of any updates or modifications through the website, email, or other means of communication. However, it is ultimately my responsibility to periodically review the agreement/waiver and policies to ensure I am aware of any changes. Continued use of The Paw Valley' services after updates or modifications to the agreement/waiver and policies shall constitute my acceptance of and agreement to abide by the revised terms. Substantial changes to the agreement will require resigning before continued use of The Paw Valley' services. If I do not agree with the updated terms, I understand that it is my responsibility to discontinue the use of The Paw Valley' services.
11. I understand that this Agreement:
a) contains the entire agreement and understanding between the Parties on everything connected with the services of The Paw Valley, and the Agreement itself; and
b) I understand and agree that, in the event any portion of this agreement shall be found to be void or unenforceable, all other portions shall remain in full force and effect.
c) I understand that The Paw Valley may update its agreement/waiver and policies at its discretion. I will find updated copies on the official website. While The Paw Valley will make reasonable efforts to notify me of changes, it's my responsibility to review the documents periodically. If I continue to use their services after updates, I accept and agree to the revised terms. If I disagree with the changes, I must stop using the services immediately.
d) supersedes any prior agreement or understanding on anything connected with that subject matter excluding the Registration Form and The Paw Valley Daycare & Boarding Policies.
e) is enforceable in conjunction with and wholly covering other completed documents related to registering and maintaining access to the The Paw Valley services, such as the Registration Form and The Paw Valley Daycare & Boarding Policies
f) By signing below, I indicate that I have read, understand and agree with all the terms laid out in this Waiver/Agreement, the Registration Form and The Paw Valley Daycare & Boarding Policies; signing below indicated you have a full understanding and agreement without qualm with the terms laid out in the three documents in conjunction.
YOU HEREBY AGREE TO EXPRESSLY AND FOREVER GENERALLY WAIVE, DISCHARGE CLAIMS, INDEMNIFY, RELEASE FROM LIABILITY, SAVE, HOLD HARMLESS AND DEFEND The Paw Valley, their invitees, owners, officers, directors, employees, contractors, volunteers, agents, representatives, lessors, and all others having an interest in any The Paw Valley party (collectively, the "Releasees") from and against ANY AND ALL INJURY, LIABILITY, CLAIMS, LITIGATION, ACTIONS, SUITS, COSTS, LOSSES, DAMAGES, ATTORNEY'S FEES, EXPENSES OR DEMANDS OF EVERY CHARACTER WHATSOEVER on account of, arising out of, resulting from or relating in any way to (i) any act or omission of the Releasees, (ii) your dog(s)' participation at any facility, on site, or otherwise. YOU AGREE THAT THIS RELEASE IS BINDING ON YOU AND YOUR SUCCESSORS, HEIRS, LEGAL REPRESENTATIVES AND ASSIGNS. YOU ALSO EXPRESSLY AND FOREVER RELEASE The Paw Valley FROM ANY DUTY TO PROTECT YOU OR YOUR DOG(S) FROM INJURY OF ANY KIND, AND AGREE THAT ANY IMPLEMENTATION OF SAFETY PRECAUTIONS BY The Paw Valley WILL NOT WAIVE The Paw Valley' RIGHT TO BE INDEMNIFIED AS PROVIDED HEREUNDER, AND SUCH PRECAUTIONS WILL NOT ALTER THIS RELEASE. YOU AGREE THAT THIS CONSTITUTES A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY OF The Paw Valley TO THE GREATEST EXTENT PERMITTED BY LAW, AND THAT IF ANY PORTION OF THIS AGREEMENT IS HELD TO BE INVALID OR UNENFORCEABLE, THE REMAINDER OF THIS AGREEMENT WILL REMAIN IN FULL FORCE AND EFFECT.

Moreover, you agree to provide The Paw Valley's staff timely updates should any information regarding your dog change, such as, but not limited to, vaccinations, behaviors, insurance status & information, emergency contacts, contact info, primary vet information, and primary address.
